Federal Post Card Application (FPCA Form 76) - Never Lived in the U.S. Instructions

What your form selections mean:

  • I am on active duty in the Uniformed Services or Merchant Marine.
    • You will receive a full ballot with federal, state, and local races.
  • I am an eligible spouse or dependent.
    • If you intend to reside in Oregon, you will receive a full ballot with federal, state, and local races.
    • If you do not intend to reside to Oregon, you are not eligible to vote and will not be registered.
  • I am a U.S. citizen living outside the country, and I intend to return.
    • You will receive a full ballot with federal, state, and local races.
  • I am a U.S. citizen living outside the country, and my intent to return is uncertain.
    • You will receive a limited ballot with only federal races.

If you are a U.S. citizen living outside the country, and you have never lived in the United States.

  • In section 1 of the form ("Who are you? Pick One”), you should check the box that reads, “I am an eligible spouse or dependent.”Form section with checkboxes; the option ‘I am an eligible spouse or dependent’ is marked with a red X and labeled ‘Required attestation'.
  • In section 2 of the form (“What is your address in the U.S. state or territory where you are registering to vote and requesting an absentee ballot?”), you should list the last Oregon residence address of the covered member of the uniformed services or merchant marine, or overseas voter, of which you are the spouse or a dependent.Form section showing address fields with a red box highlighting 'Required: Oregon Residence Address' in the Street Address field

  • In section 7 of the form (“You must read and sign this statement”), you should provide your signature and understand that it may be used to verify the signature on your ballot envelope.
