Choose a form below to request business registry document copies and certificates. Completed forms can be
submitted by mail, by fax, or in person at our service counter. We process requests in the order received.
Certificates of Existence can be ordered online and received in minutes. Log into
Oregon Business Registry and select Certificate of Existence.

Request for Copy
Get a copy of a business filing older than the printable images available on
the business’s website. Or you may request a certified copy of any document, with Authentication/Apostille if it is going out of country.
Request for Search of Old Stock Entity
Request a search of archives for records of an old entity not found in the Business Registry database.
