  1. NAME OF LIMITED PARTNERSHIP: The name as registered in Oregon.
  2. STATE OR COUNTRY OF FORMATION: Home jurisdiction.
  3. THE CORRECTION(S) TO THE APPLICATION FOR REGISTRATION…: Describe the correction(s).
  4. THE LIMITED PARTNERSHIP IS NOT TRANSACTING BUSINESS IN OREGON: Check the box to confirm.
  5. REVOCATION OF AUTHORITY: Check the box to revoke the existing Registered Agent’s authority to accept service of process. NOTE: While the Secretary of State’s office will accept service of process, they will not provide any notification.
  6. MAILING ADDRESS: Address to which a copy of documents served on the Secretary of State may be mailed in order to notify the business about the legal action.
  7. EXECUTION: This legal declaration, or a similar one, must be signed.
