2. DATE DISSOLUTION WAS AUTHORIZED: A date is required and cannot be a future date, but it can be today’s date.
3. COMPLETE EITHER SECTION 4 OR 5 BELOW. (Not both)
4. This item only applies when the corporation has not commenced business. Enter the DATE OF INCORPORATION if the stated conditions apply.
5. CHECK THE APPROPRIATE STATEMENT: One of the two options must be chosen. For the second option, complete the appropriate boxes that apply.
6. EXECUTION: This legal declaration must be signed. If another form is generated, this exact wording must be used.
