Business Name:
The business name must be identical to the name of record in the jurisdiction of the original filing. If your entity name is not available in Oregon, you must add “A Corporation of (your home state or country).’”

Activity Description:
A brief description of the primary activity of your business. Examples: ​Cafe, consulting, beauty salon, automotive repair, clothing retailer. ​

Business Email:
This address is used (rarely) for certain types of notices, but not the regular renewal notice. (This will not be a public record.) . ​

Mailing Address for Notices:
The address where notices from the Oregon Corporation Division will be sent, including renewal notices. This address may be a Post Office Box. ​

Daytime Contact:
The name and phone number of the person who can be contacted during regular business hours (8:00 AM to 5:00 PM Pacific Time) if there are questions about this filing. (This will not be a public record.)

Date of Incorporation:
This is the date your entity was originally filed in your home state or country.

Jurisdiction of Incorporation:
This is the state or country where the entity was originally filed.

Duration:
You may select perpetual or a specific date. If you select a specific date, you'll be asked to enter the latest date on which the corporation will be dissolved.

Primary Physical Location Address:
The principal office for the entity. This is the address where the corporate records are kept.

Foreign Registry Number:
This is the registration number assigned in your home jurisdiction. This number will be used to search the records of your home jurisdiction to verify the active status of the corporation.

Certificate of Existence:
This application must be accompanied by a Certificate of Existence, (or Certificate of Good Standing), current within 60 days of delivery to the Oregon Corporation Division.
