Choose a form below to request business registry document copies and certificates. Completed forms can be submitted by mail, by fax, or in person at our service counter. We process requests in the order received.

Certificates of Existence can be ordered online and received in minutes. Log into Oregon Business Registry and select Certificate of Existence.​ 

If you need an Authentication/Apostille visit the Business Registry Certificates page for instructions and submit the paper request form.

Get​ a copy of a business filing older than the printable images available on the business’s website.  Or you may request a certified copy of any document, with Authentication/Apostille if it is going out of country. ​

Request a search of archives for records of an old entity not found in the Business Registry database.​
