Choose the Right Form for the Request
Choose a form below to request business registry document copies and certificates. Completed forms can be
submitted by mail, by fax, or in person at our service counter. We process requests in the order received.
Get a copy of a business filing older than the printable images available on
the business’s website. Or you may request a certified copy of any document, with Authentication/Apostille if it is going out of country.
Request a search of archives for records of an old entity not found in the Business Registry database.
See a list of the certificates available before completing the certificate request form or ordering a Certificate of Existence online.
