Signatures

Each person specified must type their name in the signature boxes exactly as it was listed. If they are a 'Business of Record' or a 'Business not of Record', you must list the name of the person authorized to sign on behalf of the business.

The exception to this is if someone has power of attorney or is attorney in fact for ALL Signers. (See below for more information on Power of Attorney)

Power of Attorney/Attorney in Fact

An instrument in writing whereby one person, as principal, appoints another as his agent and confers authority to perform certain acts on behalf of the principal.
