  1. NAME OF LIMITED PARTNERSHIP

  2. RECORDS OFFICE ADDRESS: Must be a street address.

  3. REGISTERED AGENT: Name of the person who accepts legal service for this business.

  4. OREGON STREET ADDRESS OF THE REGISTERED AGENT: The address must be in Oregon and must match the address of the registered agent's business office. The registered office may not be a commercial mail receiving agency, a mail forwarding business or a virtual office.

  5. ADDRESS WHERE THE DIVISION MAY MAIL NOTICES: Mailing address.

  6. STATEMENT: The new registered agent has consented to this appointment.

  7. STATEMENT; The street address of the new registered office and the business address of the registered agent are identical. The entity has been notified in writing of this change.

  8. GENERAL PARTNERS ADDRESS CHANGE ONLY: Current partners address changes can be updated using this form. Adding or withdrawing general partners must be submitted as an amendment or restatement.

  9. EXECUTION: Signer declares as authorized signer, under penalty of perjury, that this document does not conceal fraudulently, fraudulently alters or misrepresents the identity of any person including officers, directors, employees, members, managers or agents.
