FOR AMENDING THE LLC

ENTITY NAME: Current name of the LLC
THE FOLLOWING AMENDMENT(S) TO THE ARTICLES…: For example, write "Article 1: new business name is …" to change the name of the business. Article numbers are found on your original Articles of Organization, but describing the amendment without the Article number is acceptable.
PLEASE CHECK THE APPROPRIATE STATEMENT: One of the two options must be chosen. (A member is an owner of the LLC.) A date is required and cannot be a future date, but it can be today’s date.
PRINCIPAL PLACE OF BUSINESS: The principal office may not be a P.O. box, commercial mail receiving agency, a mail forwarding business, or a virtual office.
INDIVIDUAL WITH DIRECT KNOWLEDGE: Any authorized representative who knows what the business does. The address may be a business or mailing address.

FOR DISSOLVING THE LLC

NAME OF LIMITED LIABILITY COMPANY: Current name of the LLC
DATE DISSOLUTION OCCURRED: A date is required and cannot be a future date, but it can be today’s date.

SIGNING THE FORM

EXECUTION: This legal declaration must be signed. If another form is generated, this exact wording must be used.
