Request for Voter Registration Cards

​Due to ongoing supply chain shortages impacting paper in the Elections Office, we are unable to fulfill all requests for voter registration cards. County elections offices may have the same limitation. ​

Registering or updating voter registration can be accomplished online with an Oregon driver's license or ID. Alternatively, you can download and print the SEL 500 Voter Registration Card in English and available here in 8 languages and large print​. Completed forms can then be sent to the appropriate county elections office.

​​​Use this form for requests of 100 or more voter registration cards. 

Direct all requests for fewer than 100 voter registrations cards to your county elections office.

Requests need approximately 5 business days to process.

The federal Help America Vote Act​ (HAVA) requires individuals to provide their Oregon DMV Driver License or Identification number or if they do not have an Oregon DMV Driver License or Identification number the last four digits of their Social Security number.
